- Please bring your Mutual Aid Association Card or health insurance card for viewing at the reception counter.
- If you have a referral please notify staff at the reception counter on level 1.
- Please notify staff if you have been involved in a car or work-related accident. Please note that patients will be responsible for full payment if the hospital is not contacted by your insurance company.
- Please complete a patient information form on arrival.
Reservations for follow-up consultations can be made at the hospital. To change reservations, please contact the relevant outpatient department.
