Complete the patient information form and present it together with your health insurance card and referral (if you have one) at the general reception counter located on level 1. If it is not your first visit to the hospital but you wish to be treated at a different clinical department, you will be required to complete the same procedures as for first-time patients. In this event, please present your existing patient registration card together with your information form. If your medical treatment is to be publicly funded or if you have been involved in a work-related accident, please present certification of public funding and/or other documentation together with the other forms.
Reservations for follow-up consultations can be made at the hospital. However, reservations for follow-up consultations cannot be made with the Otolaryngology Department. If you wish to reserve a date for your follow-up consultation, use the automatic reservation machine located on level 1.
